We prefer that you download this packet and complete it prior to your first visit so more time is devoted to getting to know you and your dental care needs and goals. You can also arrive early to your appointment and complete it in-office. Each form contains information which helps us to better serve you, or provides you with information we feel you should know.

Payment Options:
For your convenience, StarLight Dental accepts a variety of payment options; including cash, personal checks, MasterCard, Visa and Discover. For patients with dental insurance, we’ll be happy to help with the paperwork and file your claims with your insurance carrier.
CareCredit is a loan/credit card program with a range of options for patients who would like to spread out the payments for their treatment. The application process is quick and easy. Once you are approved, you can immediately begin taking advantage of CareCredit’s benefits. Some of the options offered by CareCredit include low-interest extended payment plans and 6 to 12 month interest-free plans. A member of our staff can assist you with the application process and answer any questions you may have.
Patients may apply for direct, outside patient financing for their elective procedures through CareCredit. CareCredit makes it easy to apply with a simple one page application and immediate approval online. Click the button to learn more about the program.
